Medication error

I thought on the possibility of gearing the medication error towards warfarin (Coumadin) which is something seen in the older populatation. Although, I am open to the type of medication error, it must be in something of the adult health. If your writer could think of another type of medication error I’m OK with that and must be written about in journal articles for references.

Requirements:
Research a medication error involving an aspect of pharmacology which had an impact on an individual or group of individuals specific to your pathway [Adult health]. Error must have occurred within the US and within the past 10 years. Corrective actions and the outcome of their implementation must be clearly documented in literature.

*May use a meta-analyses which has compiled errors of a similar nature and shows statistical significance.
Event(s) must be published [NOT a personal story]
Formulate a four (4) page, max 5 page [exclusive of title/ reference] APA style paper

The following MUST be included:
• Introduction of the error inclusive of specific occurrence(s), type (dose, route, etc.), significance
• Impact of the error on the individual and/ or group
• Impact of the error on the healthcare system as a whole
• Corrective actions that have been initiated in direct relation to this error
• Result(s) of corrective action

• APA Reference page of all utilized resources [journal articles, textbooks, web sources, etc.]
• Grammar and mechanics (flow) of the paper will be counted toward credit (see rubric)
